Unit: Intro to Health
Age: Middle or High School
Learning Target: The student will reflect upon their personal wellness standing. The students will write a SMART goal to help improve their lowest area of wellness.

How I teach the lesson:
I do this lesson the day after I go over each of the 7 Dimensions of Wellness with my students during one of the first few weeks of the semester. At the beginning of class after our daily routines (energizer, journal) I hand out this assessment and explain it to the students. During the class period they will complete the assessment and then write a one page reflection about it. In this reflection they must answer the following questions: what was your lowest score? Did it surprise you that it was your lowest score? Why/why not? Did any of your other scores surprise you? What are 2 practical things that you could do to raise your lowest score?
Once the students are done with their reflection they must then write a SMART goal explaining how they are going to try to raise their lowest score. At the end of the semester they will reflect to see if they actually completed their goal and why or why not.

This is a personal assessment of your overall state of wellness. This assessment will help make you aware of lifestyle choices you have made in hopes that you will maintain and/or improve your level(s) of wellness in the future.
